About Smart Disability Service Pty Ltd

We are support service provider with the National Disability Insurance Scheme (NDIS) Australia. We can help with planning, manage and execute your NDIS plan. We understand that your needs are special and that is why we have our staff trained to deliver the best services. Our staff can assist you with a different range of services such as, daily activities help, house cleaning, cooking (meal preparation), laundry, personal care, showering, accessing the community, transporting, nursing and other services.

Our support workers are well trained and all of them have an up to date National Police Check, Working with Children Check and meet the requirements. We provide an induction to our support workers so that they have the right skills and knowledge to provide quality support to our participants.

Services Catering for Kids

1. School drop offs and pickups.
2. Organise recreational activities and provide pickups and drop offs.
3. Organise and encourage kids to participate in their respective special interests activities/hobbies to build their confidence.
4. Provide personalised care to take kids to allied health service professionals e.g. speech therapist.
5. Organise and encourage kids to participate in life-long skills such as swimming.
6. Customise programs for kids in consultation with their parents so that kids are able to attend as many activities appropriate for their personal growth.
7. Support coordination services include dividing the plan of the client to balance the available allocated funds.
8. We offer 24 hour services 7 days a week throughout the year when needed.

Services For Teens

1. Development of life-long skills to uplift their lifestyle and living standards.
2. Organise driving lessons.
3. Organise programs that are suited for their personal growth and to build on their level of confidence for them to effectively cope with interacting with the general public, for example take them out to public library and spend time with them reading books of their interests. This environment will show them that there are other people reading books and it will help them to grow mentally.
4. Coach teens with elementary skills such as operating computers.
5. Organise group activities to boost their morale.
6. Provide personalised care to take teens to allied health service professionals such as physio.
7. Customise programs for teens in consultation with their families/parents so that teens are able to participate in as many activities appropriate for their personal and professional growth.
8. Support coordination services include dividing the plan of the client to balance the available allocated funds.
9. We offer 24 hour services 7 days a week throughout the year when needed.

Services For Young Adult

1. Organise driving lessons.
2. Organise social groups for young adults (camps, local community festivals, group activities)
3. Special interests (fishing, ballet, drumming, music)
4. Provide personalised care to take young adults to allied health service professionals such as physio.
5. Arrange tenancy for young adults that want to live independently.
6. Organise critical support services such as meal deliveries.
7. Support coordination services include dividing the plan of the client to balance the available allocated funds.
8. We offer 24 hour services 7 days a week throughout the year when needed.

Services Offered to Elderly Clients

1. Organise and provide shopping trips.
2. Organise and provide special appointments trips for medical, dental and allied health services.
3. Organise activities suited to their personal interests and provide transportations.
4. Organise trips to religious venues such as church and mosque.
5. Organise trips for men to men’s shed.
6. Support coordination services include dividing the plan of the client to balance the available allocated funds.
7. We offer 24 hour services 7 days a week throughout the year when needed.

General Services (For Adults)

1. Cleaning indoor and outdoor.
2. Lawn mowing to clients satisfaction.
3. Gardening services.
4. General shopping assistance, in-store and online shopping.
5. Special appointments setup with professionals such as financial planners, accountants etc and providing transportations.
6. Transport for important after hours family and recreational events.
7. Special interest entertainment for example taking them to the cinemas.
8. Group activities within their local communities organised chess games.
9. Community nurses include registered nurses and assistants in nursing.
10. Ad hoc general daily tasks around home.
11. Organise driver training.
12. Organise personal care as required/requested or 24/7 personal care availability.
13. Support coordination services include dividing the plan of the client to balance the available allocated funds.

Our Approved Services

Home Modification

Home modification incorporates design and subsequent changes to the participant’s home. Home modification design and construction includes installation of equipment or changes to building structures, fixtures to enable participants to live as independently as possible or live safely at home.

Participate Community

Assisting participants to participate actively in community, social and civic activities; includes supporting participants during these activities, and developing participants’ ability to partake in these activities.

Daily Task/Shared Living

​This registration group incorporates assistance with and/or supervising tasks of daily life in a shared living environment, which is either temporary or ongoing, with a focus on developing the skills of each individual to live as autonomously as possible. This registration group includes short-term accommodation and assistance

Development-Life Skills

​Development of daily living and life skills focuses on training and development activities undertaken by the participant or their carer to increase their ability to live as autonomously as possible, including supports that will enhance the ability of the participant to travel and use public transport independently.

Community Nursing Care

Community Nursing specialist care for participants who have high care needs requiring a high level of skill, and for the training of support workers to respond to the participant’s complex needs.

Assist Personal Activities/High

Assisting with and/or supervising personal tasks of daily life to enable the participant to live as autonomously as possible. These supports are provided by a worker with additional qualifications and experience relevant to the participants’ complex needs. These individual supports can be provided in a range of environments, including but not limited to, the participant’s own home.

Household Tasks

​Essential tasks that the participant is not able to undertake: meal preparation and delivery, house or yard maintenance, cleaning and linen.

Assist-Life Stage, Transition

​This registration group includes short and long-term supports that focus on strengthening the participant’s ability to coordinate their supports, and to assist them to live at home and participate in their community.

Specialised Drive Training

Our specialised driving instructor can deliver quality training to our participants who wants to be able to drive.

Assist-Travel/Transport

Includes rental of adapted vehicle.

Group/Centre Activity

​Assisting participants to participate in group based community, social and recreational activities.

Accommodation/Tenancy

​We help our participants with accommodation/Tenancy needs. Ask us for more information.

What is the National Insurance Disability Scheme (NDIS)?

​The NDIS is administered by National Disability Insurance Agency (NDIA) which is aimed to allow people with disabilities / special needs between the ages of 0-65 years of age to live a more ordinary life and achieve personal goals while receiving quality, flexible care completely funded by the government.
